Organizational silence can have devastating consequences, as demonstrated by:

  • USS Fitzgerald and McCain destroyer collisions where crew members knew critical information but didn't speak up
  • Korean Airways' terrible safety record in the 1970s, caused by cultural hierarchy preventing co-pilots from challenging pilots

The solution was training pilots to communicate in English, breaking down cultural barriers.

Modern organizations face similar challenges:

  • People fear risking social capital more than career capital
  • The inability to have difficult conversations predicts organizational failure
  • Junior personnel often see problems first but feel least empowered to voice concerns

Leaders must create environments where speaking up is valued and rewarded, especially empowering those closest to the front lines.
